Zia (Patrick Fugit) slits his wrists after his girlfriend Desiree (Leslie Bibb) breaks up with him. He wakes up in an afterlife that mirrors the real world but is drabber and more run-down, where cigarettes still cause cancer and no one can smile. They eventually arrive at a desert compound run by Kneller (Tom Waits), a warm, oddball figure who has built a hopeful little community out of found objects and good cheer. Kneller has a secret: he can perform small "miracles," conjuring objects out of nothing, and his dog Bugarov keeps mysteriously vanishing into thin air — a sign that leaving this afterlife may be possible.
Zia and Mikal grow close during their stay, even as Zia insists his mission is still to find Desiree. Eugene, meanwhile, falls for Nanuk (Leslie Bibb again, in a dual role — she plays both Desiree and, in some cast breakdowns, is credited only as Desiree), though the group's Kneller-camp romance subplot mainly involves Eugene pursuing a woman named Nanuk (played by Chelah Horsdal) whom he meets at the compound.
The group presses on and finally tracks down Desiree, only for Zia to realize, in finding her again, that she isn't who he wants after all — his feelings have shifted to Mikal. Desiree, revealed to be self-absorbed and unchanged by death, is no longer the answer to his longing.
The film's twist comes when Mikal is suddenly, inexplicably vanished — sucked away like Kneller's dog — suggesting she has been "sent back" to the world of the living, seemingly confirming she never belonged in this afterlife to begin with. Devastated, Zia searches for her and eventually discovers a black hole–like portal that Kneller has kept hidden, the literal doorway out of this afterlife.
